Use this guide to prepare questions before choosing a future health plan. It is general information and does not describe benefits currently available from LifeCome HMO Nigeria Limited.
Benefits and limits
A benefit schedule explains which services a plan covers and any financial, visit or treatment limits. A service mentioned in general marketing material should be checked against the formal plan documents.
Exclusions and waiting periods
Exclusions identify services or circumstances that are outside cover. A waiting period may delay eligibility for a particular benefit after membership begins. Ask how these rules affect your circumstances before enrolling.
Provider access and referrals
Check which facilities are eligible for the plan and whether referrals or authorisation are required. A provider’s inclusion in a directory does not mean that every service is covered for every member.
Effective dates and renewals
Understand when cover starts, when it ends, what payment arrangements apply and what happens at renewal. An expression of interest is not evidence that cover has started.
Privacy when someone else pays
Employers, relatives and group sponsors should understand what administrative information they can receive. Payment alone does not give unrestricted access to an adult member’s medical information.
Questions and support
Ask for a clear explanation of unclear terms and how complaints or disputed decisions are handled. General educational material does not replace professional medical advice or individual plan terms.
